The previous pages allude to a real catastrophe: the Flixborough explosion. It was caused by incompetent modification of a high pressure tubing. Today wikipedia gives a relative benign account of the actions leading to the explosion, but I remember that, by the time, science reviews gave harsher explanations involving pride, bullying and layoff threats against the aware workers.
